Nov 2, 2026: a beat, then a 13% drop
Axsome released results at 7am ET and held its call at 8am. Premarket trading ran as high as $156.14, 8.1% above the close, before the stock turned lower during the call and traded at $139.00 by 8:15am. It opened at $131.18, fell to $124.35 within the first hour, its lowest since the 52-week low of $123.50, and recovered only as far as $138.00. It was $125.84 at 10:32am on about 1.0 million shares, close to its full 20-day daily average with most of the session still to go.
The rest of the group did not move with it:
| Stock or fund | Oct 30 close | Nov 2, 10:32am | Change |
|---|---|---|---|
| Axsome (AXSM) | $144.43 | $125.84 | -12.9% |
| Biotech fund (XBI) | $155.98 | $153.98 | -1.3% |
| Neurocrine (NBIX) | $146.10 | $144.35 | -1.2% |
| Acadia (ACAD) | $20.59 | $20.44 | -0.7% |
| Jazz (JAZZ) | $215.98 | $215.82 | -0.1% |
Axsome fell about ten times as far as the biotech fund and its central nervous system peers, so this is a company move, not a sector one. More of the session's swings are in today's biggest stock moves.
After the drop, RSI(14) stood at 16, the shares sat 31% below their 50-day average and 37% below their 200-day average, and the stock was 52% below its 52-week high of $260.19. This is context, not a signal.
What the quarter showed
| Line | Q3 2025 | Q2 2026 | Q3 2026 |
|---|---|---|---|
| Total revenue | $171.0M | $218.4M | $259.7M |
| Auvelity net sales | $136.1M | $180.3M | $207.6M |
| Sunosi revenue | $32.8M | $35.8M | $48.2M |
| SG&A | $150.2M | $208.1M | $203.1M |
| Net loss | $47.2M | $51.3M | $12.9M |
The demand numbers were strong. About 302,000 Auvelity prescriptions were written in the quarter, up 14% from the second quarter, and new patient starts rose 21% from the second quarter. Among patients 65 and older, the group that takes in the new Alzheimer's disease agitation label, new starts more than tripled from a year earlier and nearly doubled from the second quarter. Net sales rose a little faster than prescriptions, which means the net price per prescription held up in the third quarter. Spending also held: SG&A fell slightly from the second quarter even after the sales force expansion for the new label.
Why the stock fell anyway: the likely reasons
Axsome did not explain the move, so these are the candidates in the record, in the order we weigh them. None of them is confirmed as the cause.
- The beat was smaller than it looked. Sunosi revenue of $48.2 million included $8.5 million of milestone revenue, against $0.5 million in the second quarter. Without it, revenue was about $251.2 million, roughly in line with the $252.7 million consensus rather than above it. Our guide on how to read an earnings release covers why one-time items get stripped out quickly.
- Fourth-quarter pricing. Investing.com reported that management expects slightly higher gross-to-net discounts in the fourth quarter because of seasonality and accrual timing. Axsome gives no revenue guidance, so a hint that net price dips is the closest thing the market had to an outlook.
- Two dates moved. The ENGAGE Phase 3 readout in binge eating disorder, listed for the fourth quarter of 2026 in the Aug 10 release, is now due in the first quarter of 2027. The smoking cessation program, listed in August as a Phase 2/3 trial starting in the third quarter, is now the Phase 3 SUMMIT trial with results in the second half of 2027.
- Questions about the new label. Analysts asked whether the Alzheimer's disease agitation launch is tracking as expected and how much of the growth comes from the larger sales force, Investing.com reported from the call. Our note on how to analyze an earnings call transcript explains why the tone of those questions can move a stock more than the numbers.
- A stock already in a downtrend. Axsome fell from $214.28 on Sep 17 to $144.43 before the report, including a 7.1% drop on Oct 23. Holders who had waited for the quarter may have used the morning pop to sell.
The bull case
- Demand is accelerating. New patient starts grew 21% from the second quarter, faster than the 14% growth in total prescriptions, which points to more growth in the quarters ahead.
- The company is close to breakeven. The net loss fell to $12.9 million, and cash fell only $6.0 million in the quarter. Axsome says its cash is enough to reach cash flow positivity, so a stock sale looks unlikely on current plans.
- A lower price for the same business. At $125.84 the market value is about $6.6 billion, about 6.4 times the annualized third-quarter revenue of about $1.04 billion. The consensus price target is $281.67, with a low of $238, though those may not reflect any cuts after today.
- A crowded calendar in 2027. ENGAGE in the first quarter, an FDA decision on AXS-12 on May 1 and four more Phase 3 readouts in the second half give the stock several chances to rerate; see how FDA decision dates work.
The risks
- No guidance. Axsome does not guide revenue, so each quarter's prescription data and net price carry the stock, and the fourth quarter is guided to a slightly lower net price.
- Delays. Two programs moved back in one quarter. Another slip in ENGAGE would weigh on the pipeline value; our guide on how to value a biotech pipeline shows how much timing matters.
- One product. Auvelity was 80% of third-quarter revenue.
- The trend. The stock has fallen in a straight line for seven weeks, and short interest is about 7.0% of the float. A first-day drop says little about the next month; see base rates in investing.
What comes next
- Weekly Auvelity prescription data through November and December, the first read on whether the 21% growth in new starts holds.
- Fourth-quarter results, not yet dated, with the full effect of the higher gross-to-net discounts.
- The ENGAGE readout in binge eating disorder, now due in the first quarter of 2027.
- The AXS-12 FDA decision, with a PDUFA target date of May 1, 2027.
